Food & lifestyle
food
ModerateAbsorption
Food may interfere with the gastrointestinal absorption of penicillamine.
What to do: Penicillamine should be administered on an empty stomach, at least one hour before or two hours after meals, and at least one hour apart from any other drug, food, or milk. This permits maximum absorption and reduces the likelihood of inactivation by metal binding in the gastrointestinal tract.

Conditions
Conditions in which Penicillamine needs care, as documented in this source.
Bone Marrow Failure Disorders
MajorThe use of penicillamine is contraindicated in patients with a history of penicillamine-related aplastic anemia or agranulocytosis.

Kidney Diseases
MajorPenicillamine is considerably excreted by the kidney, and the risk of toxic reactions to this drug may be greater in patients with impaired renal function. The use of penicillamine is contraindicated in patients with renal dysfunction. Proteinuria and/or hematuria may develop during therapy and may lead to nephrotic syndrome. Monitoring for proteinuria and hematuria is recommended. Caution should be taken and routine monitoring of renal function is recommended.

Liver Diseases
ModerateCases of intrahepatic cholestasis and toxic hepatitis have been reported with the use of penicillamine. Care should be taken and liver function tests are recommended every six months for the duration of therapy.
